Agree with @Michael Hewett that it looks like a root problem. Vine weevil would probably have been very visible when you took it out but there are other, small root pests that could be the case. Root mealybugs and such.It might be worth taking it out from the pot again and inspect the roots...

I think you might need one of the older varieties as the trend seems to be to make them compact, bright and double flowered. I'm not that familiar with the cultivars but a friend has syringa primrose which is a nice single. It is a slightly yellow white though so might not fit with what you are...
