Covering the surface roots and the trunk to that depth won't have done it any favours; but I suspect it was on it's way out anyway.I doubt it I think last year was a final fling. I would remove the tree and continue the raised bed over that area.

Yes very likely , golden leylandii and most conifers can't regenerate when cut into brown wood . I wouldn't worry about the top as you can see the sides do tend to push into the open space , it will fill in a bit faster if you raise the height of the hedge a bit.

Looking at the 2nd photo you posted my acer had a similar white patch on its bark but i had to repot it into a bigger pot as it was stressed but yours is in the ground so it looks like snail. Maybe a squirral or something has stripped the bark if there is not too much damage to the bark...
