It's often easy to tell though. I went to the optician in the summer and saw a guy who I guessed to be Nigerian, from both looks and accent. And then at the end of the consultation he started a sentence with "As we say in Nigeria, ..." ! I went away from that consultation with a perfect glasses prescription and my dry eyes diagnosed. And the dry eyes seemed to have been triggering my migraines as I haven't had any since I've been using eye drops.
All I ask is they speak reasonable English, I've had problems with a couple. |Even the nurse was struggling. And all this and we can't train our own doctors who are looking for jobs.
Indeed, in that respect I am with the doctors (I don't agree with them striking for yet another pay rise, but I do on the point where there are some getting qualified and then struggling to get a job)
In my case, the doctor had the same surname as an ex colleague of mine and I told him this and he told me that yes, he too was from Iran but had come here to escape the country after the fall of the Shah. It was a very amicable conversation. So far as I can remember he seemed quite happy, in the steady stream of “customers” he had to deal with, to be seen as a real human, individual.
Wow, let me try to answer that question. there is Medicare, which can begin when a person is 65 years old, part A and part B has lists of what they cover for hospital and rehab. a person can for about $100 a month pay for what is called co- insurance which will cover stuff part A or part B does not cover, after a year if you meet certain level of expense a percentage of it is given back to you when you complete your taxes. So far we have not been charged anything extra . People can choose to accept Medicare or they can switch to HMO insurance. Both Medicare and HMO work about the same, except some HMO's only use certain doctors. Both areas have no issue providing care in hospital etc. The big thing different than the HMO and Medicare is getting longer coverage in a hospital. HMO prefer to get the patient home quickly to get home care (which is cheaper) so I would have to fight for a few more days in the hospital by convincing them they needed it. Whereas Medicare would not need to hear from you that often to approve more days, which is why Medicare is changing now to become more like HMO, as they were taken advantage of for too many years. Looking backwards to when your younger and have a full-time job, the employer offers the employee health insurance and its free to that employee. if the employee wants their spouse covered it is an extra amount of about $200 a month. Of the years I worked full time, with any hospital visits, doctors' visits for myself or husband, nothing extra was billed. Now, if your between the age of 22 and 65 and you have a or obtain some sort of handicap then your eligible for Medicaid, as long as your financial situation warrants it. Usually coupled with that is a person who applies for SSI for funding. They also set you up for food stamps etc. A Medicaid person can go into any hospital and get anything they need without an issue, and treatment. Prior to age 22 the child can still be on their parents' insurance. Of the years I worked in a hospital setting no one was ever turned away. All these insurances also break down to levels of care needed for the person. Basically the more professional nurses /doctors that have to tend to you the higher the cost factor goes billing to the insurance companies. Like for example changing an IV versus helping one eat their dinner. That is what documentation does, it checks off what is being done for that person, then that is recorded number wise, then coverts the cost of doing that action, then add to it cost of the equipment. You asked about State differences. The only difference I know is that some states income level to qualify for Medicaid is different, off by about $200, but that only pertains to if you're wanting to leave one state and move to another state to be admitted into a nursing home, because your children live in the new state and want you closer. But the stupid part is, all you have to do is sleep one night in the nursing home to suddenly be the resident of that state. There are some interesting clauses. There is also another fun thing, Its called an ICD-10 codes. Each diagnosis, treatment, action is coded, the code corresponds to either sending directly to the insurance or telling the billing department the consumer has to pay. If the doctors office/ or lab, ect. uses the wrong code then a bill could be generated to the consumer. Folks that don't understand this will just pay the bill not knowing they can fight it. Last year, one such bill came to me, I looked up the correct ICD-10 code, called the billing office, told them, you used the wrong code, use this one, she did and poof ended that. So through human error, yes some bills can be generated. I am sure there is more stuff, but you have enough for now.

The end result has been many people in comparatively low paid jobs, but the debt of a university loan hanging over their head for 30 years.

When I was growing up near the Wash a number of neighbours and teachers used to shoot wild geese and ducks on the salt marshes. They only took a couple brace between them each time as they had to carry them back across the marsh to the cars. Seventy is a heck of a lot. The ones that really, really, really get my goat are the "trophy hunters" that go out and shoot big cats, elephants and so on, just for the hell of it. Not food, just because they can. I know where I'd like to shove their gun before pulling the trigger.
I don't understand the motivation, but I don't see it as any worse than eating meat. At least the animal lived free and (hopefully) died quickly and painlessly, unlike so many of our intensively farmed livestock.
