Sign that your compost is a bit too wet. Trouble is Squash are very susceptible to almost all insecticides. I would change the compost you are using for a non-peat one and increase the draining ability with some added Perlite.

Just had another look at mine, it looks like it happened a while ago, the holes are not fresh, and I agree it could have been some kind of caterpillar, but probably last year. Sorry pic is a bit dark.
